If your child is interested in participating in Middle School Sports during the 2023-24 school year he/she must need a completed a physical form prior to trying out for the sport. Physicals must be completed at least 3 weeks prior to tryouts. All physicals for fall sports (baseball and softball) must be completed and turned into the main office by Thursday, August 17, 2023. Physical forms can be picked up in the main office this summer, Monday - Thursday between the hours of 8 a.m. - 3 p.m. If you do not wish to use your child's pediatrician, physicals can also be scheduled at Excel Urgent Care by calling 732-874-5507.
