My name is Mrs. Evelyn Max, and I have been a certified School Nurse for over fifteen years, the past four being at Fords Middle School. I care for the medical needs of the students while they are in school. This includes first aid for school injuries, as well as taking care of those students who become sick during the school day. It is important that when a child is ill at home, that they be kept home from school that day.

My duties also include health counseling, disease prevention and developing Individual Health Care Plans for students with chronic illnesses. I screen all students for height, weight, and blood pressure on an annual basis. Additionally, sixth and eighth grade students have vision and hearning screenings, while seventh grade students have a scoliosis screening. If a follow up is needed by a physician, a referral notice is sent from my office to the parent or guardian.

Since it is sometimes necessary to contact a parent when a student is sick or injured, it is imperative to update the school when there is a change of address or phone number.
